Job summary

UF Health is seeking an Oncology Nurse Navigator to coordinate care for surgical oncology patients across the continuum, removing barriers and supporting patients, families, and caregivers with a patient-centered approach.

You will collaborate with multidisciplinary teams, track outcomes, educate patients, and serve as a central contact. Requires 3 years oncology nursing or nurse navigator experience, Associate's in Nursing, BLS, and up to 10% travel.

Qualifications

• 3 years oncology nursing or nurse navigator experience.
• Associate Degree in Nursing required; Bachelor's preferred.
• BLS certification required at time of hire.

Responsibilities

• Promotes a patient- and family-centered care environment for ethical decision making.
• Advocates for patients to promote optimal care and outcomes.
• Tracks metrics and patient outcomes and reports findings to the cancer committee.
• Serves as a liaison between this program and other areas of service.
• Educates referring physicians and the community on available services.
• Tracks patients, schedules tests, and coordinates follow-up care.
• Assesses educational barriers and provides patient education about diagnosis and plan of care.
• Coordinates entry into the EMR with physicians and team members.
• Maintains open communications with health team and patients' families.
• Supports access to psychosocial care and makes referrals as needed.
