Yale New Haven Health seeks an OMCC Nurse Coordinator to guide patients and families through care transitions, starting from referral through handoff across disease teams. You will educate families, coordinate with tumor boards, and ensure care continuity with diverse clinicians.

The role emphasizes care coordination, education, and clinical research involvement, with focus on leukemia, lymphoma, and multiple myeloma programs. Connecticut RN licensure is required.

Qualificações

• Bachelor's Degree in Nursing preferred; Master's in health-related field preferred.
• Minimum of 3 years' experience in adult inpatient services, ambulatory oncology service or operating room.
• Connecticut RN license; Oncology Nursing Society certification desirable.

Responsabilidades

• Clinical Practice: Patient/Family Assessment and ongoing data collection.
• Clinical Practice: Patient/Family Education plan development.
• Care Coordination and Patient/Family Referral: participate in treatment planning and document plans.
• Clinical Research: identify candidates for open clinical trials and coordinate with research staff.
• Clinical Program Support: oversee Intake Assistant accuracy and data completeness.
• Professional Development: set and meet annual professional development goals.
• Addendum: perform clinic nurse functions as assigned.
