Job summary

Lantern is seeking an Oncology Nurse Navigator in Dallas or New York with a hybrid in-office schedule. You will guide members from diagnosis through survivorship, delivering education, care coordination, and ongoing support.

Responsibilities include managing a full caseload, conducting comprehensive intake, educating on treatment options, coordinating multidisciplinary care, and maintaining precise documentation within Lantern's care platform.

Qualifications

• Bachelor's degree in Nursing (BSN) is required.
• Minimum 3 years oncology nursing experience.
• Active ONCC certification (OCN/CPHON/CBCN) or equivalent is required.

• Manage a full, active caseload of members with cancer diagnoses.
• Provide education and care coordination from diagnosis through survivorship.
• Educate members and families on diagnosis, treatment options, and resources.

• Perform outreach and follow-ups within defined timeframes.

Oncology Nurse Navigator

The Oncology Nurse Navigator serves as a dedicated clinical partner for members navigating a cancer diagnosis, providing personalized education, care coordination, and ongoing support throughout the treatment and survivorship journey.

Location:

Dallas, TX or New York, NY - Hybrid position - 3x in office per week.

Shift:

Monday - Friday - 9a-5pm CST (10am - 6pm EST) OR Monday - Friday 1p-9pm CST (2am -10pm EST)

Responsibilities:

• Manage a full, active caseload of members with cancer diagnoses, providing individualized navigation, education, and clinical support from diagnosis through survivorship.
• Conduct comprehensive oncology intake assessments to establish member needs, goals, and care priorities.
• Educate members and families on diagnosis, treatment options, clinical pathways, and available resources using evidence-based, NCCN-aligned and ONCC-aligned guidance.
• Coordinate multidisciplinary care by facilitating communication between members, treating providers, and the broader LCC team.
• Identify and address barriers to care including psychosocial, financial, and logistical needs; connect members with appropriate support resources.
• Complete accurate, timely documentation of all member interactions and clinical activities within Lantern's care management platform.
• Complete all outreach, follow-ups, and care coordination tasks within defined timeframes.
• Participate in call auditing processes and consistently apply feedback to maintain and improve quality of care delivery.
• Collaborate closely with the Lead ONN, LCSWs, Medical Records Specialists, Care Advocates, and cross-functional partners.
• Contribute to team meetings, case reviews, and quality improvement initiatives.
• Adhere to Lantern workflows, clinical protocols, and documentation standards at all times.

Requirements:

• Bachelor's degree in Nursing (BSN)
• Active RN license in a compact state with unencumbered ability to be licensed in all 50 states
• Minimum 3 years of oncology nursing experience
• Active ONCC certification in Oncology Nursing (OCN), Certified Pediatric Hematology Oncology Nurse (CPHON), or Certified Breast Care Nurse (CBCN) with unencumbered ability to maintain certification throughout employment
• Proficiency in Microsoft Outlook and Microsoft Office Suite
• Familiarity with NCCN guidelines and ClinicalTrials.gov
• Ability to work independently in a hybrid environment; must be Dallas or New York area based.

Compensation:

Base salary range: $90,000 - $105,000 + bonus, depending on relevant experience, location, and qualifications.
