Telephonic Case Manager RN Medical Oncology (UnitedHealth Group) – Rex, North Carolina

We are seeking a Telephonic Case Manager RN in a full-time, Monday - Friday, 8am-5pm position in your time zone. Remote work may be available in accordance with UnitedHealth Group’s Telecommuter Policy.

Primary Responsibilities

  • Make outbound and inbound calls to assess members’ current health status
  • Identify gaps or barriers in treatment plans
  • Provide patient education to assist with self-management
  • Make referrals to outside sources
  • Coordinate care across the continuum and maintain close communication with members via in-person or phone interactions
  • Support members with condition education, medication reviews and connections to resources (e.g., Home Health Aides, Meals on Wheels)

Qualifications

  • Current, unrestricted RN license in state of residence
  • Active Compact RN License or ability to obtain upon hire
  • 3+ years of experience in a hospital, acute care or direct care setting
  • Proficiency with typing and Windows-based software
  • Access to high-speed internet
  • Dedicated work area with privacy

Preferred Qualifications

  • BSN
  • 1+ years of experience in Medical/Oncology
  • Experience with discharge planning or telephonic roles
  • Background in managed care
