New York Cancer & Blood Specialists in Bohemia, NY seeks a Clinical Care Team LPN to join our oncology/hematology team. The role supports patient communication, care coordination, and appointment management across the patient journey.

The position requires NY nursing licensure and offers a Monday–Friday schedule with 7:30am–6:30pm shifts. Competitive benefits are provided as part of a comprehensive health plan.

Qualifications

• Graduate from an accredited School of Nursing and with an unencumbered New York license.
• Nursing Experience with one year of acquired clinical competence preferred.
• Excellent assessment, communication and organizational skills.
• Good computer knowledge.
• Ability to plan, organize, develop, and interpret health data, goals, and objectives that are necessary for providing quality coordination of care.
• Urology experience preferred.

Responsabilités

• Monitor and manage patient communications, coordinating timely responses and actions for patient care.
• Educate patients and answer clinical questions within the LPN scope.
• Guide patients through their care journey by arranging appointments and follow‑ups.
• Assist FMLA forms requiring provider signatures.
• Process prescription refills with healthcare providers for timely delivery.
• Use PMP for controlled substances refill follow‑ups and follow‑through.
• Coordinate with revenue cycle on authorizations and referrals.
• Arrange Peer to Peer conversations to authorize testing/medications.
• Collaborate with physicians and nurses to implement individualized care plans.
• Identify eligible patients for Chronic Care Management to support ER avoidance.
• Coordinate care with triage team for acute symptoms.
• Serve as liaison between patients and healthcare team.
• Educate patients to be part of their treatment plan.
• Maintain accurate patient records in line with health regulations.
