Job summary

New York Cancer & Blood Specialists (NYCBS) in Bohemia, NY is seeking an LPN to join our clinical care team. You will monitor patient communications, coordinate care, and guide patients through their care journey including appointments, follow-ups, and services.

The role requires NY RN license, nursing degree, and strong assessment and coordination skills. Urology experience is a plus. We offer benefits starting day one and a supportive, patient-centered environment.

Qualifications

• Graduate from an accredited School of Nursing and with an unencumbered New York license.
• Nursing Experience with one year of acquired clinical competence preferred.
• Excellent assessment, communication and organizational skills.
• Good computer knowledge.
• Ability to plan, organize, develop, and interpret health data and care goals for coordination of care.
• Urology experience preferred.

Responsibilities

• Monitor and manage patient communications, ensuring timely responses and appropriate action on messages related to patient care coordination.
• Patient outreach and education and clinical questions within the LPNs scope of practice.
• Guide patients through their care journey by facilitating appointments and follow-ups.
• Assist centralized FMLA team with completion of forms requiring provider signature.
• Coordinate prescription refills with providers and ensure timely delivery.
• Use PMP for refill requests of controlled substances with proper follow-up.
• Coordinate with revenue cycle on authorizations and referrals.
• Arrange Peer to Peer conversations to ensure timely testing and medication authorization.
• Work with physicians, nurses, and other professionals to develop individualized care plans.
• Identify qualifying patients and enroll in Chronic Care Management.
