Job summary

Mount Sinai Health System seeks a clinical pharmacist with hospital experience to lead and advance oncology/hematology pharmaceutical care. You will supervise staff, precept residents and students, participate in rounds, and develop evidence-based programs and policies to optimize drug therapy.

Responsibilities include conducting MTM, performing medication-use evaluations, and collaborating with interdisciplinary teams to improve safety and outcomes.

Qualifications

• 3 years hospital experience or equivalent with clinical experience/training.
• Doctor of Pharmacy Degree required.
• ASHP-Accredited PGY-1 required, PGY-2 preferred.
• Board Certification (e.g. BCOP) strongly encouraged.

Responsibilities

• Demonstrates the ability to perform clinical/technical/service/administrative tasks in department and hospital settings.
• Develops and implements policies and procedures to meet professional standards and improve medication safety.
• Implements formulary decisions and supports drug evaluations for the Pharmacy & Therapeutics committee.
