Responsibilities

• Provide comprehensive medication management services in collaboration with the healthcare team.
• Review patient medication history, educate patients on drug therapy, monitor outcomes and document interventions.
• Monitor drug therapy for appropriateness, dose, interactions, and safety; counsel patients on discharge medications.
• Supervise pharmacy technicians and ensure accurate dispensing and regulatory compliance.
• Support cost-effective regimens and formulary adherence; participate in quality and safety initiatives.
