OhioHealth in Columbus, OH is seeking an Oncology Clinical Pharmacist to collaborate with multidisciplinary oncology teams in the Grant Infusion Center. The role focuses on evidence-based and value-driven medication therapy, patient-specific plan optimization, and documentation of pharmacist interventions.

Responsibilities include modifying chemotherapy plans in the EHR, ensuring appropriate pre-medications, and communicating with providers. Day shift, full-time, Infusion Therapy department.

Qualifications

• Doctor of Pharmacy or PharmD degree from an accredited institution is required.
• Current Ohio pharmacist license is required.
• Board Certification in Oncology (BCOP) or eligibility to attain within 24 months is required.
• BCOP certification must be maintained during employment.
• Pharmacy Practice residency or equivalent hospital experience is required.

Responsabilités

• Review physician orders for correct drug choice, dosing, and interactions; clarify orders with physicians as needed.
• Manage and adjust patient therapy in collaboration with the oncology team.
• Document interventions and communicate recommendations to providers.
• Counsel patients and support medication adherence where applicable.
• Educate pharmacy staff and residents; supervise technicians and support personnel.
• Maintain safety, quality, and efficiency as an HRO; comply with policies.
