Responsibilities

• Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into the ambulatory care setting.
• Critically assess patient data to proactively mitigate impending changes in condition and establish interprofessional consults as appropriate.
• Leads the development and implementation of complex care plans with defined goals in collaboration with the interprofessional team and family.
• Advocates for evidence-based practice; supports journal clubs and practice changes based on new knowledge.
• Acknowledges safety-sensitive nature of the role and maintains constant alertness as required by the position.
