Responsibilities

• Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into ambulatory care.
• Leads development and implementation of complex care plans with interprofessional team and family input.
• Acts as clinical resource to nursing and support staff and ensures adherence to standards.
• Champions evidence-based practices and supports practice changes with the unit team.
• Ensures safety-sensitive responsibilities are performed with vigilance and within policy.
