Job summary

223 Aurora Medical Group, Inc. in Milwaukee, WI seeks an experienced Oncology Care Coordinator RN to join the AMG ASLMC Surgical - Hematology/Oncology team. This full-time role is 40 hours per week, Monday–Friday, 8:00 am–4:30 pm, with occasional travel with the surgical team.

Primary clinic is Aurora St Luke's, Milwaukee. The ideal candidate will hold an Associate's Degree in Nursing, RN licensure, and BLS certification, with 5+ years of clinical nursing experience in oncology and strong

Qualifications

Registered Nurse with state licensure and BLS certification.
5+ years of clinical nursing experience preferred in oncology.
Strong communication and interpersonal skills for patient and family interactions.

Responsibilities

Ensure continuity of care across settings through cancer care pathways.
Develop relationships with patients and primary physicians to navigate cancer care.
Coordinate and personalize cancer care plans with a patient-centered approach.
Perform nursing assessments focused on cancer-site care and needs.
Collaborate with teams to implement care plans and quality initiatives.
Educate patients and families on prevention and early cancer detection.
Provide community education/outreach and participate in programs.
