Job summary

Moments Hospice in Minnesota is seeking a licensed social worker to join our Social Service team on the road, providing psychosocial services to terminally ill patients and their families.

You will contact patients within 48 hours of admission, perform assessments, and contribute to care planning and bereavement support, with collaboration across disciplines. This role offers competitive pay, company car and benefits, and opportunities for growth in a mission-driven environment.

Qualifications

• Master’s degree in Social Work preferred; Bachelor's in Social Work with healthcare experience also considered.
• Minimum of two years social work experience in healthcare or social services.
• Current Social Worker license required.
• Hospice experience is a plus.
• Knowledge of community resources and eligibility requirements.
• Understand hospice philosophy and death/dying.
• Strong organizational and communication skills.
• Valid driver's license and auto insurance required.

Responsibilities

• Provide psychosocial services to terminally ill patients and families.
• Contact patient, family, and caregiver within 48 hours of admission.
• Case manager duties.
• Document psychosocial assessment within 24 hours.
• Regularly assess psychosocial needs of patient and family.
• Assess pre-bereavement and anticipatory grief needs.
• Implement Plan of Care social service goals and referrals.
• Participate in weekly interdisciplinary team meetings.
• Assist with funeral arrangements if requested.
• Provide counseling during terminal illness and at time of death if requested.

Qualifications:

• Master’s degree in Social Work preferred, Bachelor's degree in Social Work with a minimum of 1 year of healthcare experience.
• Minimum of two years social work experience in a healthcare setting or social services agency
• Current Social Worker license (required)
• Hospice experience a plus
• Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.)
• Understand Hospice philosophy, principles of death/dying
• Strong organizational and communication skills

Responsibilities:

• Provide wide range of psychosocial services to terminally ill patients and their families.
• Responsible for making contact with patient, family, and caregiver within 48 hours of admission.
• Case Manager duties.
• Psychosocial assessment must become part of patient's chart within 24 hours of completion.
• Regularly assess the patient and family psychosocial needs.
• Regularly assess the pre-bereavement/anticipatory grief needs.
• Implement the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement care.
• Regularly re-evaluate the patient's and family's psychosocial needs.
• Participate in weekly interdisciplinary team meetings.
• Assist with making funeral arrangements if requested.
• Provide counseling during the process of the terminal illness and at the time of death if requested
