Berkshirejobs is seeking a CTLP Case Assistant to support the Community Transition Liaison in nursing facilities, serving as an onsite contact for residents, families, health care proxies, and NF staff during transitions to the community.

The role requires a high school diploma with one year of related experience, strong interpersonal and communication skills, and proficiency in Microsoft Office. Travel is involved and benefits include health and dental insurance.

• Support the Community Transition Liaison in coordinating transitions from NF to the community.
• Travel as needed to nursing facilities to facilitate smooth transitions.
