Job summary

Growing Hope is seeking a Program Coordinator for Leaphart Place, a transitional housing program for young adults ages 18-21. This on-site role coordinates daily operations, goals, and community supports and ensures program records are complete and current.

The position balances resident support with outreach, safety, and accountability, including rent tracking, admissions, and partnerships with DSS caseworkers. A bachelor’s degree and relevant experience are required.

Qualifications

• Bachelor's degree in social work, human services, psychology, sociology, education, public administration, community development, or a related field.
• At least three years of experience in youth or young-adult services, transitional housing, independent living, homelessness services, or related settings.
• Demonstrated ability to build rapport with young adults while maintaining clear expectations and boundaries.
• Experience supporting goal development, monitoring progress, coordinating services, and helping individuals navigate barriers.
• Strong organization and follow-through with multiple deadlines.
• Ability to maintain accurate confidential documentation and enter data into required systems.

Responsibilities

• Meet residents on a consistent schedule to support transitions and independence.
• Develop individualized goals and action steps for graduation from Leaphart Place.
• Monitor progress, document accomplishments, and identify next steps.
• Provide coaching, reminders, and resource navigation.
• Coordinate move-ins, admissions, and orientation.
• Maintain resident and program records and HMIS data as required.
