Job summary

Gilbert Center for Family Medicine in Gilbert, AZ is seeking an experienced Nurse Practitioner or Physician Assistant to join our primary care team. This Advanced Practice Provider will work independently and with physicians to deliver high-quality, patient-centered care in a busy family medicine setting.

The role requires an NP/PA license in Arizona, DEA certification, and 2+ years of primary care experience. On-site, full-time with competitive salary and benefits.

Qualifications

• Current, valid Nurse Practitioner or Physician Assistant license in Arizona
• Current federal DEA certificate to prescribe controlled substances
• Minimum 2 years of Advanced Practice Provider experience in a primary care setting
• BLS certification (must be obtained within 30 days of hire)

Responsibilities

• Conduct comprehensive patient evaluations and manage acute and chronic conditions
• Collaborate with physicians and clinical staff to coordinate patient care
• Prescribe medications and order appropriate diagnostic testing
• Document patient encounters accurately in the EMR
• Make appropriate referrals for conditions beyond scope of practice
• Contribute to quality improvement, mentorship, and team development
