Job summary

Janus RX Pharmacy is seeking a Care Coordinator to act as a liaison between the pharmacy team and partner facilities. This role works on-site at partner facilities to support medication management and continuity of care.

The Care Coordinator will build relationships with clinic staff and consumers, coordinate refills, resolve insurance barriers, and promote Janus RX services to onboard and retain patients.

Qualifications

• Active pharmacy technician license required; national certification through PTCB or ExCPT a plus.
• 1–2 years of pharmacy technician experience or two years of healthcare experience.
• Ability to read and communicate effectively in English.
• Strong communication skills and ability to build relationships with providers and consumers.

Responsibilities

• Build strong relationships with Janus RX partners including consumers, case managers, and clinic staff.
• Coordinate prescription refills between providers’ offices and delivery to patients.
• Assist with and resolve insurance barriers to medications.
• Promote Janus RX services to onboard and retain consumers and patients.
• Coordinate delivery of medications and ensure accurate insurance information is charged.
• Educate patients on the ability to fill prescriptions through Janus RX Pharmacy.
• Navigate prior authorizations and Patient Assistance Programs as needed.
• Organize collection of prescriptions and scheduling of deliveries.
• Perform monthly prescription refill calls to monitor compliance and insurance status.
• Review reporting cycles and interpret data to drive actions with clinic staff and consumers.
• Conduct weekly reporting on medication discrepancies and dose changes.
