Qualifications

• Active LPN license in Illinois.
• Nursing degree/diploma from an accredited program.
• Strong assessment and organizational skills.
• Detail-oriented with excellent communication abilities.
• Adherence to Healthcare Code of Ethics.
• Familiarity with Joint Commission Standards.
• Knowledge of State and Federal confidentiality regulations.

Responsibilities

• Conduct comprehensive nursing assessments, including ASAM forms.
• Provide routine care at various levels of care.
• Identify treatment needs and integrate findings into treatment plans.
• Complete nursing documentation per facility and state requirements.
• Review medication logs and assist the medical department.
• Administer medications as directed by the medical provider.
• Facilitate medication education and integrate issues into plans.
• Collaborate with patients, families, and referrals to update progress and plan post-discharge care.
• Apply clinical judgment in crisis situations and perform risk assessments.
• Tailor nursing approaches to meet diverse patient needs.
• Build rapport and maintain trust with patients through strong interpersonal skills.
