Job summary

Parc Center for Disabilities is seeking a dedicated Case Manager to develop and implement Individual Service Plans for adults with developmental disabilities. You will work closely with Direct Support Professionals to ensure health, safety, and meaningful programming.

This on-site role blends administrative and hands-on duties, including documenting progress, coordinating vocational opportunities, and meeting waiver requirements.

Qualifications

• Minimum of 3 years’ experience working with intellectually and developmentally disabled adults.
• Experience in case management and/or vocational services.
• Knowledge of Medicaid waiver requirements and compliance standards.
• Strong organizational, analytical, and documentation skills.
• Ability to manage multiple priorities in a fast‑paced environment.

Responsibilities

• Develop and implement Individual Service Plans and structured goal programs.
• Enter and maintain programming within the Therap system.
• Monitor goal implementation and provide periodic written updates.
• Provide leadership and direction to Direct Support Professionals.
• Ensure overall health, safety, and well‑being of assigned participants.
• Coordinate vocational opportunities with Community Supports Supervisor.
• Maintain accurate documentation including activity, progress, incident, attendance, and statistical reports.
• Meet with participants, families, and outside agencies as needed.
• Participate in team meetings and program planning.
• Provide crisis management and behavioral intervention when necessary.
• Assist participants with daily living activities as needed.
• Take rotating on-call responsibilities to cover staffing and scheduling needs.
