Job summary

ColumbiaCare Services, Inc. is seeking an On Call Supportive Housing Associate for programs in the Southern Oregon area, including Medford and Ashland.

The role motivates adults with serious mental illness to participate in skills training and treatment activities, while helping with housing applications, health care arrangements and daily living support. Evening and overnight shifts are available; on-site stay earns a $75 extra on nights slept on-site, with additional language differential for

Qualifications

• Must be eligible for QMHA registration.
• Crisis de-escalation training provided as part of the program.
• Ability to drive and pass a DHS background check.

Responsibilities

• Facilitate access to community resources and health care arrangements.
• Assist tenants with housing applications and move-in planning.
• Provide medication administration, money management and symptom monitoring.
• Perform housekeeping duties and support socialization, self-help and problem solving.
• Maintain accurate clinical documentation and participate in team care.
