Job summary

ColumbiaCare Services in Southern Oregon seeks an On Call Supportive Housing Associate to support adults with severe mental illness across Medford and Ashland. You will help with health care access, housing applications, and daily living tasks, while promoting independence.

The role requires QMHA eligibility or enrollment assistance within 30 days, crisis de-escalation training, and compassionate patient-centered care on a direct care team.

Qualifications

• Must be eligible for QMHA registration or assist with registration within 30 days of hire.
• Bachelor’s degree in Behavioral Science, Psychology, Counseling, Social Work or related field or 3+ years behavioral health experience.

Responsibilities

• Motivate adults with serious mental illness to participate in skills training and treatment activities.
• Facilitate access to community resources and assist tenants with healthcare, housing applications, and move-in arrangements.
• Administer medications and manage money, symptom monitoring, housekeeping; support problem solving and socialization.
