Phoenix Home Care & Hospice in Springfield, MO is seeking an experienced On-Call Registered Nurse to support our hospice team. This role delivers timely, skilled end-of-life care in patients’ homes during nights and weekends.
You’ll respond to urgent needs, perform routine and as-needed visits, document accurately, and collaborate with an interdisciplinary team to provide compassionate, patient-centered care. A Missouri RN license is required.
Qualifications
- Missouri RN license or compact license required.
- Ability to work independently in patients' homes.
- Hospice nursing experience preferred.
Responsibilities
- Respond to urgent needs of hospice patients and families nights and weekends.
- Complete routine and as-needed home visits across Springfield, MO area.
- Triage urgent situations and coordinate with hospice physicians.
- Collaborate with interdisciplinary hospice team and caregivers.
- Maintain accurate clinical documentation and comply with standards.
- Deliver professional, compassionate, patient-centered hospice care.
- Participate in ongoing training and quality initiatives.
