Job summary

Community LIFE is seeking an On-Call Registered Nurse (RN) to provide after-hours and weekend care in participants' homes. The role offers full-time pay for working every other week in the Bedford service area, with overnight on-call shifts from 3:30 PM to 8:30 AM.

Responsibilities include assessing episodic illnesses, delivering prescribed nursing interventions, educating participants and caregivers, documenting changes, and coordinating with the interdisciplinary team.

Qualifications

Graduate of an accredited nursing program.
Valid PA driver's license.
Minimum 1 year of experience with frail or elderly populations.
Experience in patient and caregiver education preferred.

Responsibilities

Provide after-hours nursing care and support to older adult participants.
Assess and manage episodic illnesses in collaboration with the on-call provider.
Deliver prescribed treatments and nursing interventions.
Educate participants and caregivers on care plans and health management.
Document all care and changes in condition promptly and accurately.
Coordinate care and communicate with the interdisciplinary team.
