Job summary

Amedisys is seeking an on-call Registered Nurse for a 7 on/7 off schedule to provide high-quality home health care. The role emphasizes continuity of care, patient education, and collaboration with the care team to ensure patients heal where they are most comfortable.

Qualifications

• Current RN license valid in the assigning state(s).
• Minimum 1+ year nursing experience.
• Current CPR certification.

Responsibilities

• Perform initial nursing assessment on new admissions.
• Coordinate and revise the plan of care and ensure continuity of patient care.
• Regularly report patient progress to clinical manager, physician and care team.
• Maintain patient care records with accuracy and timeliness.
• Document pain/symptom changes and educate families on patient condition.
