Job summary

Horizon House in Seattle, WA seeks an on-call Resident Care Coordinator to ensure resident-centered care across shifts. You will lead assessments, care planning, and coordination with families and providers while guiding direct care staff to maintain quality and safety.

Qualified LPN/RN in WA with CPR and food handler permits, plus MS Office proficiency, are required. On-call role with shift differentials and a strong team culture offered.

Qualifications

• Must hold an active WA LPN or RN license in good standing.
• CPR certification or ability to obtain within 30 days of hire.
• Food Handler's Permit or ability to obtain within 14 days of hire.
• Proficiency with Microsoft Office applications (Outlook, Teams, Word, Excel).

Responsibilities

• Coordinate and oversee resident care by assessments, care plans, monitoring well-being, and ensuring services meet needs.
• Partner with residents, families, healthcare providers, and community resources to coordinate services and transitions.
• Provide leadership and support to direct care staff through coaching, training, and guidance.
• Promote quality, safety, and regulatory compliance through documentation, audits, and infection prevention efforts.
• Support admissions, staffing, emergency preparedness, and continuous improvement projects.
