Registered Nurse (RN) Job Summary

As a HomeCare Registered Nurse (RN), you will provide comprehensive nursing services to individuals, families, and groups in their homes. You will be responsible for the overall coordination of the Plan of Care and proficient in all components of the nursing process. This role reports directly to the Clinical Supervisor.

Key Responsibilities:

• Provide skilled nursing care to patients in their homes, including admission and discharge.
• Teach, demonstrate, supervise, and evaluate care provided by peers, HHAs, family members, and significant others.
• Assess patients' physical, psychosocial, emotional, cultural, spiritual, and discharge needs according to their age and developmental level.
• Develop and implement individualized plans of care.
• Utilize effective oral and written communication skills, ensuring completeness and accuracy of documentation.
• Evaluate and adjust plans of care as necessary.
• Demonstrate technical skills and operational knowledge of patient care equipment and procedures, including venipuncture for IV medication administration.
• Recognize and appropriately intervene in unsafe and life-threatening situations.

Required Skills and Qualifications:

• Graduate of an accredited school of nursing.
• Registered Nurse (RN) license issued by the state of Connecticut.
• Valid Connecticut Driver’s License and proof of current insurance coverage.

Preferred Skills and Qualifications:

• Two years of recent medical/acute care or critical care experience as a registered nurse.
• Home Health Care specific competencies.
