Job summary

Hospice of Holland, Inc. is seeking an On-Call Registered Nurse (RN) to provide in-person and telephonic hospice care. The role supports a multidisciplinary on-call team to maintain patient comfort and quality of life.

The RN will respond to patient and family needs, coordinate care, document, and ensure equipment and supplies are available. Michigan licensure and valid driver’s license are required.

Qualifications

• Graduate of an accredited school of nursing with current RN licensure in Michigan.
• Two years of recent clinical experience preferred; homecare or hospice experience desired.
• Strong critical thinking and clinical judgment.
• Knowledge and/or experience in pain management for end-of-life care.
• Reliable transportation with valid driver’s license and auto insurance.

Responsibilities

• Respond to patient, family/caregiver needs in person or by phone.
• Identify issues and determine best clinical response for comfort and quality of life.
• Initiate interventions and monitor outcomes.
• Collaborate with on-call team including other RNs, social worker, aide, physician, and supervisor.
• Educate patients and family members.
• Document and chart care.
• Ensure necessary medical equipment, supplies, and services are available.

On-Call Registered Nurse (RN) - Hospice Care

The On-Call Registered Nurse (RN) provides on-call ski lled nursing care for hospice patients. This is done in person and telephonically. The On-Call Registered Nurse (RN) has the support of other on-call team members. Together, the team works to bring outstanding end of life care to our patients and their caregivers.

Position Responsibilities

• Responding to family/caregiver and patient needs, either in person or telephonically
• Identifying what the issue/need is and formulating the best clinical response to maintain the patient’s highest level of comfort and quality of life
• Initiating appropriate interventions and monitoring the outcomes
• Collaborating with other members of the on-call team including other on-call registered nurses, the on-call medical social worker, the on-call hospice aide, the on-call physician, and the on-call supervisor
• Educating patients/family members
• Documenting/charting
• Ensuring necessary medical equipment, supplies, and services are available

Position Qualifications
