Job summary

Commonheart is seeking an RN for a full-time in-home hospice/home health role in Wisconsin. You’ll travel to patient homes, provide direct care, and partner with families and the care team to deliver compassionate, quality services.

The position offers a 7 on/7 off schedule, with evening shifts and weekend rotation, plus full benefits and a company car. Experience in home hospice or home health is preferred, and an active Wisconsin RN license is required.

Qualifications

• RN license in Wisconsin is required
• Current, valid driver’s license is required
• Minimum of 1 year experience in home hospice or home health preferred

Responsibilities

• Provide in-home hospice or home health care to patients and families
• Build genuine one-on-one connections with patients and observe changes in condition
• Collaborate with the care team to ensure high-quality, compassionate care

Job Status

• Full Time

Salary

• $75K to $80K, based on experience

Schedule

• 7 on / 7 off, Monday-Sunday
• Monday - Friday: 4PM - 8AM
• Weekend: Saturday at 8AM to Monday at 8AM

Location

Hit the road without the extra costs—this role includes travel and a company car is provided.
