Job summary

Commonheart is seeking an experienced Registered Nurse for home health and hospice care in Wisconsin. You will travel to patients’ homes, providing compassionate nursing support and coordinating care with a dedicated team.

The position follows a 7 on / 7 off schedule with 7 on Sunday through Saturday. Hours include Monday–Friday 4 PM–8 AM and weekend coverage from Saturday 8 AM to Monday 8 AM, with a company car provided for all travel.

Qualifications

• RN license in Wisconsin required.
• Current, valid driver’s license required.
• Minimum of 1 year experience in home hospice or home health preferred.

Job Status

Salary

• $75K to $80K, based on experience

Schedule

• 7 on / 7 off, Monday-Sunday
• Monday - Friday: 4PM - 8AM
