Job summary

Presbyterian Homes & Services (PHS) is seeking an On-Call Occupational Therapist at Lake Minnetonka Shores in Minnesota. Weekday shifts with hourly on-call premiums, and a chance to work across TCU/SNF, Assisted Living, Independent Living, Home Health, and Outpatient.

The role emphasizes resident-centered care, thorough assessments, and coordinated discharges to support quality of life. You'll join a supportive, mission-driven nonprofit with opportunities for continuing education and growth.

Qualifications

• Graduate from an accredited Occupational Therapy program.
• Current licensure in the state(s) of practice as required.
• Compatibility with PHS's mission and operating philosophies.
• Ability to read, write, speak and understand English to communicate with all customers.

Responsibilities

• Conduct a comprehensive assessment & evaluation using subjective and objective measures.
• Develop treatment plans and programs in accordance with physician orders.
• Provide quality treatments, actively progressing goals to improve quality of life.
• Screens and educate residents/clients, families, caregivers, and staff.
• Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.
• Provide accurate and timely documentation & billing in electronic medical record.
• Demonstrate understanding of payer sources and regulatory requirements.
