Job summary

Trinity Health is seeking an On-Call Registered Nurse to provide skilled end-of-life nursing care for hospice patients in the Columbus, OH service area. The role focuses on after-hours coverage (5:00 PM–8:00 AM, 7 days a week) with comprehensive assessments, symptom management, and coordination with the interdisciplinary team.

Ideal candidates hold an Ohio RN license with CPR certification and hospice/home health experience.

Qualifications

Active Ohio RN license.
Current CPR certification.
Hospice, home health, oncology, critical care, ER, or med-surg nursing experience preferred.
Strong assessment, communication, and autonomous decision-making skills.
Ability to travel throughout the greater Columbus market and participate in on-call rotations.

Responsibilities

Perform comprehensive patient assessments and evaluate changes in condition.
Provide symptom management and end-of-life nursing care.
Respond to after-hours, weekend, and holiday patient needs.
Coordinate care with physicians, hospice leadership, facilities, and interdisciplinary team members.
Complete admissions, routine visits, crisis visits, General Inpatient (GIP), and Continuous Care (CC) visits as assigned.
Educate patients and caregivers regarding disease progression, medications, and comfort measures.
Ensure accurate and timely EMR documentation and compliance with hospice regulations.
Participate in quality improvement initiatives, team meetings, and ongoing professional development.
