Job summary

Heartland Hospice seeks an On-Call Registered Nurse to provide compassionate hospice care to patients with life-limiting illnesses in homes and facilities. The RN triages after-hours calls, makes urgent visits, assesses symptoms, and implements care plans focused on comfort and dignity.

Responsibilities include pain and symptom management, medication administration, patient and family education, and timely documentation.

Qualifications

• Must hold an active RN license.
• Experience in hospice or palliative care preferred.
• Strong communication and compassionate patient care.

Responsibilities

• Triage after-hours calls and determine urgency.
• Make urgent home or facility visits.
• Assess pain and symptoms; adjust care plans.
• Administer medications per orders and monitor effects.
• Educate patients and families; provide emotional support.
