Job summary

St. Croix Hospice in Maple Grove, MN is seeking an experienced Registered Nurse On-Call to join our 24/7 care model. You’ll provide after-hours nursing visits, respond to triage, and support admissions with clinical expertise and compassionate care.

The 7 on / 7 off overnight schedule offers work-life balance while delivering high-quality end-of-life care. RN licensure in MN and CPR certification required; hospice/home care experience preferred.

Qualifications

• Graduate of an accredited nursing program (Associates, Bachelors, or Masters).
• 1+ years of recent acute care experience; hospice or home care preferred.
• Current RN license in state of practice and CPR certification.
• Respond to triage calls and provide after-hours nursing visits for symptom management, admissions, and care plan updates.
• Develop and adjust individualized care plans based on evolving needs.
• Administer medications and treatments as prescribed, offering education and support to patients and caregivers.
• Maintain timely and accurate clinical documentation of care, outcomes, and communication.
