Concho Hearts Hospice PRN On Call RN 7 days on / 7 days off On Call cycle
Area: Pilot Point / Denton / Sherman area

Our team of Nurses, Aides, Chaplains, Social Workers, Volunteers, and Doctors provides symptom management, and patient and family support while coping with an end-of-life illness. Job Summary:
This is an on-call only position, it will be a cycle of 7 days on call and 7 days off on call. It is a full-time position.
Primary functions are to administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient, family, and referring agency; and assume the responsibility for coordination of care.

RN Responsibilities:
  • Preparing and adjusting patient care plans as needed.
  • Providing loved ones and primary caregivers with patient care information, end-of-life education, and emotional support.
  • Maintaining and updating medical charts, patient files, and health records.

RN Job Qualifications:
  • Education:
    • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Licensure:
    • Current license as a registered nurse (RN) and/or by the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC), current Driver's License.
  • Experience:
    • Two years of experience as a Registered Nurse in a clinical care setting preferred, hospice preferred.
  • Skills:
    • Nursing skills as defined as generally accepted standards of practice.
    • Good interpersonal skills.
    • Specialized skills preferred in palliation and end-of-life.
    • Proof of current CPR. Must read, write, and comprehend English.
  • Transportation:
    • Reliable transportation.
    • Valid and current auto liability insurance.
  • Environmental and Working Conditions:
    • Works in patients' homes in various conditions, possible exposure to blood and bodily fluids and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.
  • Physical and Mental Effort:
    • Prolonged standing and walking required, with the ability to lift up to 50 lbs and move patients.
    • Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psycho-social needs.

Transportation:
  • Reliable transportation.
  • Valid and current auto liability insurance.

Environmental and Working Conditions:
  • Works in patients' homes in various conditions, possible exposure to blood and bodily fluids, and infectious diseases; ability to work a flexible schedule.
  • Ability to travel locally.
