Job summary

Good Shepherd Hospice is seeking an On Call Nurse (RN or LPN) to serve patients and families in Garnett, Kansas and surrounding communities. You will perform nursing assessment, planning, and care to maximize comfort and health in accordance with the Interdisciplinary Plan of Care.

Responsibilities include coordinating with care managers and physicians, documenting care plans, and providing education on medication administration, personal care, and safety.

Qualifications

• RN or LPN licensure in the state where care is provided.
• Hospice experience preferred.
• Maintains confidentiality of patient records.
• Demonstrates integrity and teamwork in care plans.
• Willingness to pass background checks and have a valid driver’s license.

Responsibilities

• Receive patient assignments and coordinate with care managers and physicians.
• Communicate with patients and families, and provide education on care and safety.
• Maintain up-to-date patient records and Plan of Care.
• Assist in pain management, personal care, and safety activities.
• Participate in team meetings and care conferences as needed.
• Provide support at time of death and offer bereavement assistance.
