Qualifications

• Must be a Registered Nurse with current licensure in the state.
• Graduate of National League for Nursing accredited school of nursing.
• Maintains a current CPR certification.
• Must be a licensed driver with insured automobile in good working order.
• Minimum of two years’ experience, at least one in public health, home care or hospice nursing.
• Self-directed and able to work with minimal supervision.
• EMR knowledge (Homecare/Homebase/PointCare) preferred but not required.

Responsibilities

• Complete initial, comprehensive and ongoing assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Contribute to the plan of care and revise as needed.
• Administer medications and treatments per physician’s plan of care.
• Counsel patient and family in nursing and related needs.
• Maintain documentation per procedures and standards.
• Ensure safety and continuity of care across the interdisciplinary team.
