Position Summary

Next Care Hospice is seeking a compassionate, dependable, and patient-focused Licensed Vocational Nurse (LVN) to provide on-call nursing support to hospice patients and their families. The On-Call LVN will respond to patient needs, perform nursing visits as assigned, assist with symptom management, and provide support to patients experiencing changes in condition.

The ideal candidate is comfortable working independently in the field, communicating with the interdisciplinary hospice team, and providing compassionate care to patients and families during challenging moments. This position plays an important role in ensuring timely nursing support, continuity of care, and patient comfort.

Key Responsibilities

• Respond to on-call patient needs and perform nursing visits as assigned, including evenings, nights, weekends, and holidays according to the established schedule.
• Assess and report patient symptoms and changes in condition within the LVN scope of practice and agency policies.
• Provide nursing care and assist with symptom management in accordance with the established plan of care and RN or physician direction, as applicable.
• Respond to patient and family concerns with professionalism, compassion, and sensitivity.
• Communicate promptly with the supervising RN, Director of Patient Care Services, and other members of the interdisciplinary team regarding patient status and care needs.
• Assist with medication administration, treatments, and other nursing interventions as permitted by California law, the plan of care, and agency policy.
• Document nursing visits, patient observations, interventions, and communications accurately and in a timely manner.
• Provide education and support to patients and caregivers within the LVN scope of practice.
• Follow hospice policies, applicable state and federal regulations, infection prevention procedures, and patient confidentiality requirements.
• Coordinate with the clinical team to ensure appropriate follow-up and continuity of care.

Qualifications

• Current, active California Licensed Vocational Nurse (LVN) license in good standing.
• Previous hospice, home health, or other relevant nursing experience preferred.
• Strong communication, organization, and time-management skills.
• Ability to work independently in patients' homes and respond reliably to assigned on-call visits.
• Compassionate bedside manner and the ability to support patients and families during end-of-life care.
• Reliable transportation, a valid driver's license, and current auto insurance.
• Ability to complete required clinical documentation and communicate effectively with the interdisciplinary team.

Preferred Qualifications

• Previous experience with hospice patients and end-of-life symptom management.
• Familiarity with electronic medical records and hospice documentation.
• Experience providing nursing care in home-based settings.
