Responsabilidades

• Provide individual and group bereavement counseling for Hospice patients, families and community members.
• Document all client contacts and care plans accurately per policy.
• Demonstrate timely contact with clients and document progress, including additional contact attempts when needed.
• Provide counseling services with flexible scheduling, including some weeknights, weekends and camps.
• Collaborate with the Interdisciplinary Team to ensure the best possible services for customers.
