Job summary

UnitedHealth Group's LHC Group seeks an On Call Registered Nurse to deliver and direct nursing care to patients in their homes, coordinating care with physicians and the care team to ensure continuity and quality. The role includes participation in on-call rotations with additional pay for visits performed while on call.

Requirements include an active RN license in the practicing state and a valid driver's license with reliable transportation.

As the On Call Registered Nurse you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. You will also coordinate total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. In addition to competitive compensation, this role offers additional pay for participating in the on-call rotation and completing patient visits while on call.

Primary Responsibilities:

• Provide high-quality clinical services within scope of practice and infection control standards.
• Coordinate care with other members of the patient/client's care team from admission to discharge.
• Complete clinical nursing assessments per federal/state program requirements and payer needs.
• Ensure patient/client eligibility and medical necessity for services as defined by payer and agency policy.
• Develop and revise individualized plans of care/service plans with other community providers.
• Ensure plan of care frequency and duration meets patient needs and initiate revisions with physician approval.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualifications:

• Current and unrestricted RN licensure in the state of practice
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation.

Preferred Qualifications:

• Minimum of one year of RN experience
• Current CPR certification or ability to obtain within 90 days of employment
• Able to work independently
• Good communication, writing, and organizational skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $39.45 to $59.15 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
