Anchor Health seeks a Hospice Team Nurse to provide professional nursing care to patients, utilizing the nursing process and collaborating with the interdisciplinary team. The role emphasizes compassionate care and communication with families and physicians to ensure optimal patient outcomes.

The nurse will be accountable to the Director of Patient Care Services and coordinate patient needs with community physicians and family members, after hours and on weekends as required.

Qualifications

• Licensed RN with current California license.
• Must have a valid driver’s license and insured automobile.
• Experience in hospice or end-of-life care is a plus.

The after hours nurse supports our families needs after hours. After hours is qualified as after 5PM and weekends. The professional nurse is accountable to the Director of Patient Care Services for implementing the patient plan of care and communicating each patient needs to the Interdisciplinary team. Requirements

Patient Care

The Hospice Team Nurse provides professional nursing care to patients by utilizing all elements of nursing process.

Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
