Job summary

Outreach Community Ministries Inc. in Carol Stream, IL, is hiring a Full‑time Older Adults Program Director to serve as the clinical lead for senior services.

The role provides direct clinical care, crisis intervention, and care coordination while overseeing program quality, supervision, and partnerships. This position supports ethical practice, risk management, and strategic development of comprehensive senior services.

Qualifications

• Master's degree in social work (MSW) and active clinical license (LCSW or LCPC).
• Experience delivering clinical services to older adults.
• Sensitivity to needs of low‑income, culturally diverse populations.
• Nursing or medical field experience is a plus.

Responsibilities

• Provide clinical assessment, intervention, and case management for older adults.
• Conduct mental health and wellness assessments and develop service plans.
• Deliver crisis intervention and ongoing clinical monitoring.
• Provide advocacy and care coordination with medical, behavioral health, housing and community resources.
• Maintain timely and ethical clinical documentation.
• Supervise and mentor interns (BSW/MSW).
• Coordinate and evaluate a diverse calendar of programs promoting healthy aging and engagement.
• Manage program operations, schedules, transportation, volunteers, facilities, budgets, and resources.
• Collaborate with community agencies to strengthen referral pathways and continuity of care.
