Job summary

Integrated Services for Behavioral Health (ISBH) is a community-minded organization delivering behavioral health services across Southeast and Central Ohio. The OhioRISE Care Coordinator will organize a multisystem team to support youths at home, in school, and in the community, emphasizing family voice and coordinated care.

The role requires effective communication, adherence to HIPAA, and the ability to travel within service area to meet care plans.

Qualifications

• Experience providing services and/or support to children and families connected to behavioral health or related public sector human services.
• Two years of experience in a coordinated supportive services or care coordination role that required high efficiency and attention to detail.
• Experience with people with autism spectrum disorders and developmental disabilities is preferred.

Responsibilities

• Develop, lead, and facilitate team meetings for the young person and their identified team.
• Provide care coordination within the family’s home, community, or preferred location.
• Maintain required documentation to access and maintain OhioRISE benefits.
• Travel within assigned service area as needed.
