Active Home Care's Office Nurse assists the Home Care team with the clinical components of the program, focusing on onboarding new Personal Care Aides and supervising patient-centered care in the home. The Office Nurse provides in-office orientation, evaluates skills in the home, and ensures staff skills meet client needs to promote safe patient care and ongoing support.

Responsibilities

What you'll do: In addition to working with an AMAZING team, your responsibilities will include:

  • Visiting new referrals to assess the ability to safely provide in-home care
  • Reviewing clinical components and post-test results with new hires during orientation
  • Providing clear clinical materials to Personal Care Aides, with remediation and additional training as needed
  • Supervising new aides post-orientation in the home, ensuring their skills match client needs
  • Reporting any potential concerns in the client's home immediately
  • Scheduling ongoing quarterly visits and communicating changes to the Care Team
  • Submitting all clinical documentation promptly and accurately
  • Creating and signing off on client Care Plans during yearly and quarterly visits
Qualifications

Qualifications include:

  • Active RN or LPN license
  • Registration with the applicable state examining board
  • At least 21 years of age
  • Solid understanding of home care and field staff disciplines
  • Flexibility in staffing and scheduling
  • Ability to build strong relationships
  • Excellent written and verbal communication skills
  • Ability to cover an assigned area of responsibility
  • Successful background check and a good driving record
