Job summary

RHA Health Services, LLC is hiring a Licensed Practical Nurse (IDD LPN) for an office-based, full-time role in the Chattanooga area. The position requires regular visits to group homes within Chattanooga for resident assessment and clinical support.

This is a structured, day-time schedule (Monday–Friday, 8:30am–5:00pm) with on-site responsibilities in group homes. The ideal candidate has an LPN license, current CPR certification, and the ability to work with an interdisciplinary team.

Qualifications

• Graduation from an accredited licensed nurse program.
• Current LPN license with no restrictions to practice nursing.
• Current CPR certification or eligibility to certify for CPR.
• Possess credentials required by federal or state law.
• Effective communication with Interdisciplinary Team Members and families.
• Ability to work independently, be organized, and prioritize tasks.
• Must be at least 18 years of age.

Responsibilities

• Provide holistic nursing care to individuals with IDD in residential settings.
• Observe health data, report needs to the registered nurse.
• Coordinate preventive and routine medical care for people supported.
• Document nursing care provided and assist with on-site clinics.
• Provide health-related trainings to unlicensed staff per regulations.
• Conduct medication pass observations and ensure staff competency.
• Perform quality assurance checks on health-related tasks.
• Communicate with physicians, pharmacies, and the Interdisciplinary Team.
• Advocate for people served and help develop community supports.
