Job description

Prime Home Health is growing and we are seeking skilled and compassionate Certified Occupational Therapy Assistants (COTAs) to join our dynamic team on a PRN basis. In this role, you will provide vital occupational therapy services to patients in their homes in all areas of San Antonio, promoting their independence, functional abilities, and overall well-being. Your expertise will be crucial in helping individuals regain and maintain their daily living skills, ensuring a higher quality of life within the comfort and familiarity of their own homes.

Responsibilities:
  • Conduct comprehensive assessments of patients' functional abilities, including physical, cognitive, and psychosocial aspects, through careful observation, interviews, and standardized assessment tools.
  • Collaborate with Occupational Therapists to develop personalized treatment plans, setting appropriate goals and interventions to address patients' unique needs and maximize their independence.
  • Implement therapeutic activities and exercises aimed at improving patients' motor skills, coordination, balance, strength, endurance, and cognitive function.
  • Assist patients in learning and practicing adaptive techniques, using adaptive equipment, and modifying their home environment to facilitate safe and independent performance of activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
  • Educate patients and their caregivers on proper body mechanics, energy conservation techniques, and strategies for preventing further injury or disability.
  • Document patients' progress, treatment interventions, and outcomes accurately and in a timely manner, adhering to organizational and regulatory standards.
  • Collaborate with interdisciplinary healthcare team members, including physicians, nurses, social workers, and other therapists, to ensure coordinated and holistic care for patients.
  • Maintain a professional and compassionate demeanor while delivering high-quality services to patients, respecting their cultural, social, and personal values.
  • Stay updated with the latest research, evidence-based practices, and advancements in occupational therapy, incorporating new knowledge into clinical practice.
  • Adhere to ethical and professional standards set forth by regulatory bodies and uphold patient confidentiality at all times.
Qualifications:
  • Minimum one year experience in providing occupational therapy services in an adult in-home health setting.
  • Valid certification as a Certified Occupational Therapy Assistant (COTA) from an accredited program.
  • Current Texas license as a COTA.
  • Working knowledge of Kinnser/Wellsky EHR and other applications such as Slack, which we use for internal communications.
  • Valid CPR BLS certification.
  • Strong knowledge of occupational therapy principles, techniques, and best practices.
  • Familiarity with various conditions, disabilities, and injuries that require occupational therapy interventions.
  • Excellent interpersonal, communication, and organizational skills.
  • Ability to work independently, manage time effectively, and prioritize tasks in a dynamic environment.
  • Compassion, empathy, and patience when working with patients facing physical or cognitive challenges.
  • Proficiency in documentation and record-keeping, with attention to detail and accuracy.
  • Reliable transportation and willingness to travel to patients' homes within designated geographical areas.
