Job summary

Evolve Therapy Services is hiring compassionate Certified Occupational Therapy Assistants for an in-house Skilled Nursing Facility in Oklahoma City, OK. This role supports plan development and resident care in a collaborative setting.

Eligibility requires an Associate’s degree in Occupational Therapy, active OTA licensure in the state(s) of practice, and ongoing continuing education to maintain licensure. Full-time, part-time, or PRN options available with competitive pay and benefits.

Responsibilities

• Conduct regular screenings to determine the best intervention for each patient.

• Communicate evaluation findings and recommendations to residents, families, and team members.
• Establish individual therapy schedules with the care team.
• Document and bill for occupational therapy services per plan of care.
• Perform other duties to effectively treat patients and support CMS standards.

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
