Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Phenix City, AL. The role supports ongoing resident care through hands-on therapy per the plan of care, under supervision of a licensed therapist.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Perform treatment plans and follow goals from plan of care.
• Communicate evaluation findings and plans to residents, families, and interdisciplinary team.
• Establish resident therapy schedules with team members.
• Document and bill for occupational therapy services following treatment plans.

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications
