Position Details

Job Title: Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type: PRN

Location: 1905 W Hart Rd, Beloit, WI 53511, USA

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in the facility, ensuring the highest quality of resident care in accordance with federal, state, and local standards, guidelines, and regulations, under the direction of the Administrator.

Key Responsibilities
  1. Conduct regular screenings to determine the best intervention or treatment for each resident based on scope of practice.
  2. Implement treatment plans, following short- and long-term goals established by supervising therapists.
  3. Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and the interdisciplinary team.
  4. Establish individual therapy schedules in collaboration with team members.
  5. Perform therapy sessions according to the treatment plan, ensuring proper documentation and billing of occupational therapy services.
  6. Perform additional duties as assigned to effectively treat patients.
  7. Comply with all CMS documentation standards and maintain accurate point-of-care documentation daily.
Qualifications
  • Associates Degree in Occupational Therapy.
  • Active OTA licensure in the state(s) of practice is required.
