Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in South Sioux City, NE. This PRN role supports resident care under the supervision of a licensed therapist, ensuring high-quality occupational therapy services.

Responsibilities include screening, treatment plan execution, documentation, and collaboration with the interdisciplinary team. Active OTA licensure in state(s) of practice is required, with ongoing continuing education

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification required.

Responsibilities

• Conduct regular screening of residents to identify optimal intervention.
• Perform treatment plans according to established goals with supervision.
• Interpret and communicate evaluation findings to residents, families, and interdisciplinary team.
• Establish individual resident therapy schedules with the team.
• Document and bill Occupational Therapy services per CMS standards.
• Perform other duties to effectively treat patients as assigned.
• Comply with CMS documentation standards on a daily basis.
