Job summary

Evolve Therapy Services is hiring a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in South Sioux City, NE. This PRN role focuses on planning, directing, and delivering OT services in accordance with regulatory standards and facility directives.

Responsibilities include screening residents, developing treatment plans with goals, communicating findings to families and the team, and supervising COTA as needed.

Qualifications

• Degree in Occupational Therapy is required.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Plan, organize, develop, and direct Occupational Therapy services in the facility.
• Screen residents regularly to determine best intervention or treatment.
• Evaluate residents and develop treatment plans with goals and methods.
• Communicate evaluation findings and recommendations to residents, families, and the care team.
• Establish therapy schedules with other team members and ensure documentation.
• Provide therapy according to plan and handle billing for OT services.
• Supervise COTA as needed and ensure compliance with reimbursement practices.
